This form may only be used to cancel a course up until the start of the second session.
A $75 course cancellation fee
will apply, and the campus administrator will be notified.
Cancellation requests made after participant attendance in a second Spalding course session will forfeit the full
tuition cost.
I have read and understand the above conditions regarding course cancellation and hereby authorize Spalding
Education International to cancel me from the course I have listed in the course cancellation request form.
